What type of supportive documentation is accepted for my Recertification Appeal?
If you completed continuing education towards your recertification/renewal, you must attach the related records of completion in addition to the appeal form.
If you are submitting an appeal due to an extenuating circumstance, below are examples of acceptable documentation. Please include the applicable supportive documentation in addition to your completed appeal form.
- Death of family member: Death certificate, Obituary or similar
- Medical/Illness: Doctor's note
